*If you are requesting a document that is not listed above, please upload the document with your submission or email it to sixthsa@ucsd.edu. You will be prompted to sign a Consent/Release of Student Information Form via DocuSign, so please check your email following your submission.

Note: Please submit your request as early as possible. Requests could take approximately up to 7 - 10 business days from request to notice of completion. We will do our best to submit by your document deadline, but it is not guaranteed if the due date is less than 7-10 business days.
